Confidential workplace incident report

Use this form to report workplace incidents, inappropriate behaviour, safety concerns, client misconduct, bullying, sexual harassment, boundary concerns, and any matter that may affect health or safety.

 

Reporting Instructions

  • Complete this form as soon as possible after the incident or concern is identified.

  • Where there is an immediate threat to safety, notify a manager or director first and contact emergency services if required.

  • Attach or list any supporting evidence such as screenshots, CCTV references, emails, messages, photographs, or witness details.

  • Submit the completed form to the designated manager, director, or confidential reporting contact.
